Bright beginnings early learning center
All enrollment forms will be provided upon enrollment in person. We require enrollment forms to be completed accurately and in full as well as submitted a minimum of 5 business days before the first day of attendance.
Instructions For Filling Out Paperwork
- Please do not use white out
- Please do not leave any blank entries and do not write “same as above”, as all information is required
- Please write “n/a” if entry is not applicable
- Please return all paperwork at minimum of 5 business days before the child starts with us so that we have the proper amount of time to review the paperwork and ask any questions for clarifications.
- We are notaries and can notarize the Emergency Medical Release form.
